2024 Maternal Child Health (MCH) Objective Supplement, F-01611 (08/2023)

Thank you for taking the time to review and select maternal child health objectives to implement in your communities in 2024. We are grateful for the work you do you every day to promote maternal and child health around the state and are looking forward to reviewing what your agency will be focusing on this upcoming year.

Instructions: 

  1. In the Division of Public Health Grants and Contracting (GAC) System, local health departments and Tribal health agencies need to select 2024 MCH objectives, assign a dollar value, and click the notification button.  
  2. New for this year, local health departments and Tribal health agencies will use Alchemer to complete a Maternal Child Health (MCH) Objectives Supplement for each objective they have selected. This process will replace the former GAC Supplement.   
  3. MCH Objectives Supplements must be submitted by November 3, 2023. 
  4. Contract monitors will review the MCH Objective Supplements submitted in Alchemer and notify agencies of approval or requested edits.  
  5. When MCH Objectives Supplements are finalized and approved, both contract monitors and local health departments or Tribal health agencies will enter negotiation notes in GAC. Contract monitors will then sign off in GAC. 
  6. Negotiations must be completed by November 10, 2023.  
Once you start your MCH Objective Supplement, you will not be able to save it and come back later. To help with this, you can download a print version of the 2024 MCH Objectives Supplement. When you are ready to submit, come back to this online form, copy and paste your responses, and submit.
