Optional and Customized Questions

Please let us know which of the optional questions you would like to include in the Communications & Media Use Audience Outlook Monitor survey when you send it out. You can view the survey on the cohort webpage where the optional questions are marked with a green tag.

Note that if you do not select a question or grouping of questions below, it will not be on the survey when you send it. None of the questions on the survey will be mandatory and respondents can skip any number of questions and still complete the survey.

At the end of this form you will be automatically sent an email with a pdf of the answers you submitted for your records.

Questions 15-16 allow you to ask survey respondents which radio stations they listen to and how frequently. What answer items would you like to use for these questions? You may list up to nine.

Format answers as "[Name/Call sign] [Frequency] [AM/FM]"
e.g., "WDET 101.9 FM"

Make sure to designate your local NPR associate as such if included. Note that "SiriusXM," "Other station (not listed)," and "None of the above" are mandatory and will be included in addition to any options you list below.
 
Questions 19-20 allow you to ask survey respondents which local TV stations they watch to and how frequently. What answer items would you like to use for these questions? You may list up to ten.

Format answers as "[Name] [Channel] ([Affiliation])"
e.g.. "CBS Detroit Channel 62 (CBS Affiliate)"

Please prioritize those that might reach target segments of your audience and do not include cable channels. Note that "Other station (not listed)" and "None of the above" are mandatory and will be included in addition to any options you list below.

Question 40 allows you to ask survey respondents which of your proprietary digital channels or media sources they listen to or watch. What answer items would you like to use for these questions? You may list up to ten.

Format answers with your name and then the media source.
