Quarterly reporting tool for the FY24 School Substance Use Prevention grants. Every quarter, you will be asked to report the number participants for each strategy. You will also be asked to report the number of unduplicated students that participated in any of the prevention activities every quarter. For Quarter 4, you will need to report the total number of students in grades 6-12. You can upload up to 5 documents each grant quarter. During site visits, we will talk more in-depth about the prevention work happening in your school district/supervisory unions.
