Environmental Health Complaint Form

The Livingston County Health Department responds to restaurant complaints and other issues involving unsanitary environmental health conditions in Livingston County, Michigan. Use this form to submit a safety and/or sanitation complaint regarding restaurants, body art facilities, public swimming pools, public beaches, septic/sewer system failures, etc. Common complaints we receive include restaurant employees not wearing gloves while handling food, sewage surfacing from a failing septic system, and tattoos being performed in someone's home. All fields marked with asterisk * are required.

• If your complaint is concerning a health or environmental emergency, call 911.
• We do not handle customer service complaints such as rude staff, incorrect orders, or slow service time. Please report these incidents to the facility's management.
