Papātai Oranga Wellbeing Survey: Epilepsy NZ

Here at Epilepsy NZ, our mission is to improve the quality of lives of New Zealanders living with epilepsy and unexplained seizures. To do this, we are asking for your help to find out what is most important to your wellbeing, so we can tailor our services to serve you better.

The survey should take about 15-20 minutes to complete.

Epilepsy NZ has partnered with independent measurement experts Huber Social and Melde to assess the impact of its organisation and services. This survey will be used to understand your overall wellbeing, including taha hinengaro (psychological health); te taha wairua (spiritual health); te taha tinana (physical health); and te taha whānau (family health) as well as topics such as how supported, safe, and empowered you feel.
Your privacy
In this survey, you will not be asked to provide your name, number or any other contact information; all survey results remain anonymous. All data remains de-identified to protect your privacy and no individual results will be shared. Huber Social and Epilepsy NZ will never disclose your personal information to other parties without your consent, except in response to legal requirements.

If you choose to, you may enter the draw to win one of three Ultimate Ears UE BOOM 3 Wireless Portable Bluetooth Speakers! (They normally retail for $159 each). To enter the draw you will be asked to supply your email address. You will be asked for this after you have submitted the survey. Your contact details will be stored separately to your responses. All contact details will be removed once the measurement period has closed.

Your data will be stored securely in the Huber Social Wellbeing Database; in addition to this study, your data may be aggregated with larger datasets to develop broader insights into sector-wide wellbeing and needs. Please click here for more information about Huber Social’s Data Privacy Policy.
