The purpose of this form is for Dane County facilities to report COVID-19 outbreaks to Public Health Madison & Dane County and request assistance if needed.
Public Health only needs to be notified of an outbreak event
once. An outbreak event is considered to be over after 28 days have passed with no new cases.
