The Self-Care of Heart Failure Index (SCHFI) - Union Health HF program

The Self-Care of Heart Failure Index (SCHFI) measures self-care with three scales: 
  • Self-care maintenance
  • Measuring behaviors to maintain Heart Failure stability 
  • Symptom perception 
  • Measuring monitoring behaviors 
  • Self-care management 
  • Assessing the response to symptoms.
Think about how you have been feeling in the last month or since we last spoke as you complete these items.
