SHEA Mentor Scholar Award:
The SHEA Mentor Scholar Award is intended to honor an individual recognized for his/her dedication and excellence in mentoring trainees in infection prevention, healthcare epidemiology, or antibiotic
stewardship. The award consists of $1,500 and a plaque.


Eligibility:
• Nominees must be recognized for their impact on education, infection prevention, hospital
epidemiology, or antibiotic stewardship.
• Nominees must be members of SHEA in good standing.
• Prior recipients are ineligible for this award.

Nomination Procedure:
• A nomination letter from a SHEA member outside of the nominee's institution
• A support letter from the nominee's mentees
• Nominee's CV
• Application

Scoring:
• Nomination Letter: Summarize the nominee's contributions to infection prevention, healthcare epidemiology, or antibiotic stewardship The letter should also highlight the     candidate's dedications to education and mentorship.
• Nominee’s CV: Candidate should express a dedication to education and mentorship in infection prevention, special accomplishments in the field, and a track-record of mentoring.
• Mentee letter(s): Candidate has been especially accomplished and has contributed to mentoring the future of the field in a significant and special way.
SHEA Senior Scholarship Award
The SHEA Senior Scholarship Award recognizes the work of an investigator/practitioner in infection prevention, healthcare epidemiology, or antibiotic stewardship in practice for 15 years or more. The
award consists of $1,250 and a plaque. Selection will be based on the breadth and quality of the nominee's contributions to infection prevention, healthcare epidemiology, or antibiotic stewardship.


Eligibility:
• Member of SHEA in good standing.
• In practice for 15 years or more (starting from the time of completion of fellowship training).

• Nominee's CV
• Nominee's Letter (One page maximum)

Scoring:
• Nomination Letter: Summarize the nominee's contributions to infection prevention, healthcare epidemiology, or antibiotic stewardship. It should also demonstrate the nominee’s scholarship achievements.
• Nominee’s CV: Candidate should be involved in a professional organization related to infection prevention, healthcare epidemiology, or antibiotic stewardship at the local (chapter) or national level.
• Nominee’s letter: The nominee’s letter should highlight career interest & goals in prevention and special accomplishments in the fields of infection prevention, healthcare epidemiology and/or antibiotic stewardship with evidence of publications and significant impact of their work.
SHEA Pediatric Scholarship Award
The SHEA Pediatric Scholarship Award is intended to recognize the work of an investigator in pediatric infection prevention, healthcare epidemiology, or antibiotic stewardship. The award consists of $1,250 and a plaque. Selection will be based on the breadth and quality of the nominee's contributions to the field of pediatric infection prevention, healthcare epidemiology, or antibiotic stewardship.

Eligibility:
• Member of SHEA in good standing.
• Significant contributions to pediatric infection prevention, healthcare epidemiology, and
antibiotic stewardship

• Nominee's CV
• Nominee's letter (One page maximum)

• Nomination Letter: Summarize the nominee's contributions to infection prevention, healthcare epidemiology, or antibiotic stewardship. It should also demonstrate the nominee’s scholarship achievements.
Nominee’s CV: Candidate should indicate that they are involved in a professional organization related to pediatric infection prevention, healthcare epidemiology, or antibiotic stewardship at the local (chapter) or national level.
Nominee’s letter: Candidate demonstrates career interest & goals in pediatric infection prevention, healthcare epidemiology, or antibiotic stewardship, special accomplishments in the field with evidence of publications and significant impact of their work.
SHEA Mid-Career Scholarship Award
The SHEA Mid-Career Scholarship Award recognizes the work of an investigator/practitioner in infection prevention, healthcare epidemiology, or antibiotic stewardship in practice for 5-15 years. The award consists of $1,250 and a plaque. Selection will be based on the breadth and quality of the nominee's contributions to infection prevention, healthcare epidemiology, or antibiotic stewardship.

Eligibility:
• Member of SHEA in good standing.
• In practice for 5-15 years (starting from the time of completion of fellowship training).
• Prior recipients of SHEA Investigator Award (pre-2015) or SHEA Pediatric Investigator Award are ineligible for this award

Nominee’s CV: Candidate is involved in a professional organization related to infection prevention and healthcare epidemiology at the local (chapter) or national level.
Nominee’s Letter: Candidate demonstrates career interest & goals in infection prevention and/or antibiotic stewardship, special accomplishments in the field with evidence of publications and significant impact of their work.
SHEA International Scholarship Award
The SHEA International Scholarship Award recognizes the work of an investigator/practitioner in infection prevention/healthcare epidemiology in practice outside of the United States and Canada. The award consists of $1,750 and a plaque. Selection will be based on the breadth and quality of the nominee's contributions to infection prevention, healthcare epidemiology, or antibiotic stewardship. 

Eligibility:
• Member of SHEA in good standing.
• In practice outside of the United States and Canada

Scoring:
• Nomination Letter: Summarize the nominee's contributions to infection prevention, healthcare epidemiology, or antibiotic stewardship. It should also demonstrate the nominee’s scholarship achievements.
Nominee’s CV: Candidate is involved in a professional organization related to infection prevention and healthcare epidemiology at the local (chapter) or national level in their country or region.
Nominee’s Letter: Candidate demonstrates career interest & goals in infection prevention, special accomplishments in the field with evidence of publications and significant impact of their work.
SHEA Advanced Practice IP Award
The SHEA Advanced Practice Infection Prevention (IP) Award is intended to recognize professional accomplishments and contributions to the science of infection prevention, healthcare epidemiology, or antibiotic stewardship by an experienced professional in the field. The award will be given annually to a non-physician infection preventionist to support his/her continuing education and professional development. The award consists of $1,250, and a plaque. The Awards Committee will recommend the finalist to the SHEA Board of Trustees for approval.

Eligibility:
• Nominee must be a professional actively involved in infection prevention, healthcare epidemiology, or antibiotic stewardship for at least 10 years.
• Nominee must be a SHEA and/or Association for Professionals in Infection Control and
Epidemiology (APIC) member.
• Nominee must be a recognized expert and show evidence of significant contributions to infection prevention, healthcare epidemiology, or antibiotic stewardship research, and have created, delivered, promoted, or influenced infection prevention, healthcare epidemiology, or antibiotic stewardship practice, or influenced public perception, attitudes, and awareness.
• Prior recipients are ineligible for this award.

Scoring:
Nomination LetterSummarize the nominee's contributions to infection prevention, healthcare epidemiology, or antibiotic stewardship. It should also demonstrate the nominee’s scholarship achievements.
Nominee’s CV: Candidate is a recognized expert in the field and involved in a professional organization related to infection prevention and healthcare epidemiology at the local (chapter) or national level.
Nominee’s Letter: Candidate demonstrates career interest & goals in infection prevention, special accomplishments in the field including publications, invited presentations at national/international meetings, etc. The candidate should also indicate that they have been active in infection prevention and control/healthcare epi for at least 10 years and show evidence of contributions to infection prevention and control research (i.e. publications, presentations, etc.)
SHEA Antibiotic Stewardship Scholarship Award
The SHEA Antibiotic Stewardship Scholarship Award recognizes the work of an investigator/practitioner in antibiotic stewardship that has been involved in stewardship for 5 years or more. The award consists of $1,250 and a plaque. Selection will be based on the breadth and quality of the nominee's contributions to antibiotic stewardship programs and research. The Awards Committee will recommend the finalist to the SHEA Board of Trustees for approval.

Eligibility:
• Member of SHEA in good standing. (includes Member-in-Training)
• In practice for 5 years or more (starting from the time of completion of fellowship training).

• Nominee's CV
• Nominee's Letter (One page maximum)

Scoring:
• Nomination LetterSummarize the nominee's contributions to antibiotic stewardship. It should also demonstrate the nominee’s scholarship achievements.
• Nominee’s CV: Candidate demonstrates strong career interest in antibiotic stewardship, special accomplishments in the field. Candidate is involved in a professional organization related to antibiotic stewardship at the local (chapter) or national level.
 Nominee’s letter: Candidate expresses career interest & goals in antibiotic stewardship, special accomplishments in the field including publications, invited presentations at national/international meetings, etc. The candidate should also highlight their track record in research leading to significant practice innovation.
