Thank you for your interest & participation in the efforts of the Dance/USA Task Force on Dancer Health for professional dancers. This information clarifies the process for becoming involved in our initiatives for the 2023-2024 Season:
- The registration form MUST be completed before any screening documents are distributed. The form MUST be filled out every year, regardless if the form has been filled out previously.
- Please complete one form per organization. If you have more than one company/division within the organization, complete the information required for all companies/divisions that will be screened. Note: ALL boxes do NOT need to be completed if not appropriate for the company.
We strongly suggest that you print out this blank sheet to gather the information you will need to complete this online form. You will not be able to save drafts or incomplete entries. Note: This information is collected for demographic purposes only. It gives the Task Force information regarding the companies utilizing the screen.
