Thank you for taking the time to submit your complaint and/or report to our office. We take ALL complaints and reports very seriously.
Once this is completed and submitted, our office will receive notification right away. Our administrative team will review and enter the information right away, then forward the information to the corresponding health inspector for that county. The health inspector will then investigate the situation. Please be as specific as possible in your text answers..
If submitting an illness complaint, it will be forwarded to our Epidemiology team who will then forward back to us once all information has been gathered.
