12-hr SBIRT Training (In-person Offering)

As we strive to provide both quality and value to our community that we serve, our public health response to substance use can offer an opportunity to make a measurable difference. SBIRT (Screening, Brief Intervention, Referral to Treatment) guides all levels of prevention; primary, secondary, and tertiary.  Recommended substance use screening tools can be found at the SAMHSA website (http://www.integration.samhsa.gov/clinical-practice/screening-tools) and will be reviewed during training.

The person’s responses on the screening tool will support the SBIRT conversation that follows. The screening tool can be completed by the person or by designated staff.  Our “way of being” when discussing their substance use will impact our relationship with them and their subsequent motivation to consider any change.

A caring and non-judgmental approach builds trust and safety. “Telling people what to do” and “fixing them” will often decrease their willingness to consider change. This is where the skills of Brief Intervention offer an effective alternative. Brief intervention is guided by Motivational Interviewing.
  1. Ask permission prior to sharing the screening results. Support autonomy.
  2. Provide the results of the screen, including the recommendations that would be made based on the score/answers.
  3. Ask “What do you make of this?”
  4. Listen carefully for their perspective and notice if there is any indication that they are considering a change. This is the time to reflect and affirm.
Primary prevention will be the focus when the screen indicates that the person is not using substances at all or may be using alcohol at low risk levels. This is an opportunity to provide affirmation for their choice. “You are choosing not to use substances at all” or “to stay within the recommendation for “low risk drinking”. “This is a choice that supports your health and wellness.” This is especially important for teens and young adults

Secondary prevention occurs when the person is using substances and/or engaging in risky drinking but does not meet criteria for a serious substance use disorder. “Your screen indicates that you are drinking at risky levels and could benefit from drinking less. What do you make of this?”. This person may benefit from a few scheduled “brief interventions”.

Tertiary prevention is implemented when the person’s score suggest they have a serious substance use disorder (SUD). The recommendation will be substance use disorder treatment. This is the “referral to treatment” that is the last “RT” in SBIRT. NYS OASAS website offers a list of current SUD providers and some information regarding availability of appointments.

People benefit when SBIRT is approached as a prevention tool that is as important as all other public health prevention activities such as immunization, mammogram, and colon-rectal screening.
