Vaccines for Children (VFC) Program Enrollment Form

New York City (NYC) Vaccines For Children (VFC) program

Dear Provider,

Thank you for your interest in enrolling with the New York City (NYC) Vaccines For Children (VFC) program. This form is no longer accepting new enrollments. Providers can enroll in the VFC Program through the CIR Facility Manager website. Please see the requirements for enrollment into the NYC VFC program below. If your practice meets these requirements please proceed to the CIR Facility Manager website to begin the enrollment process. 

 

VFC Participation Information:

The VFC program is a federally funded program that provides vaccines at no cost to children who might not otherwise be vaccinated because of inability to pay. The CDC buys vaccine at a discounted rate for distribution to registered VFC providers. Children who are eligible* for VFC vaccines are entitled to receive those vaccines recommended by the Advisory Committee on Immunization Practices (ACIP).

To participate in the NYC VFC program, providers must serve children from birth through 18 years of age who fall under any of the following categories:

  • Medicaid eligible
  • Uninsured
  • Underinsured: A child who has commercial (private) health insurance but the coverage does not include vaccines, a child whose insurance covers only selected vaccines (VFC-eligible for non-covered vaccines only), or a child whose insurance caps vaccine coverage at a certain amount. Once that coverage amount is reached, the child is categorized as underinsured
  • Native-American or Alaskan Native
  • Enrolled in Child Health Plus B (CHPlus B)

Please note that the following children are NOT considered underinsured:

  • Children whose insurance covers a portion of the cost of the vaccine but not the full amount. In this case providers should consider renegotiating the contract with the insurance company
  • Children whose insurance covers vaccines but have a high deductible and whose deductibles have not been met

In addition to serving VFC eligible populations, providers must NOT be included in the Medicare/Medicaid Restricted and Excluded Individuals or Entities list.

VFC-enrolled provider sites must participate in the VFC Immunization Quality Improvement for Providers (IQIP) program, if selected. To learn more about the IQIP program, please visit CDC: (IQIP) Immunization Quality Improvement for Providers.
