Individual Placement and Support Award Nominations

The Individual Placement and Support (IPS) Wisconsin State Leadership Team is seeking award nominations for the 2024 Wisconsin Individual Placement and Support Annual Meeting. 

DVR Vocational Rehabilitation Champion Award
For being committed to the principles of the Individual Placement and Support model, collaborating with employment specialists and treatment team members, and advocating for employment and education goals. This award may go to anyone with DVR, regardless of role.

Employer Champion Award
For being committed to collaboration with Individual Placement and Support, inclusion, and recovery in the workplace. This award may be awarded to up to three employers.

Employment Specialist Champion Award
For being an Individual Placement and Support employment specialist committed to the principles of the Individual Placement and Support model, engaging with job seekers, collaborating with other professionals, and advocating for employment and education goals.

Individual Placement and Support and County Treatment Team Collaboration Award
For vocational agencies and county treatment teams showing commitment to team integration, zero exclusion, collaboration, and supporting the employment and education goals of shared consumers. In order to be eligible for this award, the employment specialist must attend, on average, at least four treatment team meetings per month.

Individual Placement and Support and DVR Collaboration Award
For vocational agencies and DVR Workforce Development Areas showing strong commitment to collaboration and supporting the employment and education goals of shared consumers. In order to be eligible for this award, employment specialists must have, on average, at least one scheduled meeting with the DVR vocational rehabilitation counselors each month.

Individual Placement and Support Champion Award
For being committed to the principles of the Individual Placement and Support model and demonstrating focus on quality improvement and implementation of Individual Placement and Support. This award may go to anyone involved in Individual Placement and Support, regardless of role.

Resilience Award
For agencies showing resilience in the face of extreme struggles, perseverance, open to change and evolution, and continued commitment to Individual Placement and Support.

Transformation Award
For being an agency that made a significant transformation (examples: implemented a new program, onboarded lots of new employment. specialists, executive team showed new/innovative support, grant use, significant increase in fidelity score, serving more treatment teams, etc.)
 

Please complete this form to nominate a person, agency, or collaboration for an award. Nominations are due by June 18, 2024.

The 2024 Wisconsin Individual Placement Support Annual Meeting will be held July 16 from 9:00 a.m. to 4:00 p.m. in Wausau. Registration is required. There is no registration fee. Registration will open no later than June 3.
