2023-24 DoD Allocation Adjustment Form

Please use this form to request DoD allocation adjustments for the 2023-24 SY. Requests will be processed after the 10th of each month. If your request is submitted after the 10th, it will be reviewed the following month.

Please note: This form is to be completed by the Designated Official only (Program or Ordering Contact per CNPweb). June 10th will be the last and final date for DoD adjustments for the current school year, so please plan accordingly.
