Temperature Excursion Incident Report

Introduction

In the event of a temperature excursion, be sure to

Quarantine the vaccines and mark "DO NOT USE" to inform staff
Contact the vaccine manufacturers
Complete and submit this report to the New York City Department of Health and Mental Hygiene, Bureau of Immunization, Provider Quality Assurance Unit.
VFC, VFA, or Pediatric Flu Vaccines that are wasted or spoiled should be reported to the Citywide Immunization Registry
MPOX Vaccines that are wasted or spoiled should be reported to the Facility Manager (FACMAN) Application

This report serves as a record of the incident, the steps taken to determine vaccine viability, and the disposition of the affected vaccines. Keep this report for your records.
