Temperature Excursion Incident Report

Introduction

In the event of a temperature excursion, be sure to
  • Quarantine the vaccines and mark "DO NOT USE" to inform staff
  • Contact the vaccine manufacturers
  • Complete and submit this report to the New York City Department of Health and Mental Hygiene, Bureau of Immunization, Provider Quality Assurance Unit.
  • VFC, VFA, or Pediatric Flu Vaccines that are wasted or spoiled should be reported to the Citywide Immunization Registry 
  • MPOX Vaccines that are wasted or spoiled should be reported to the Facility Manager (FACMAN) Application
     
This report serves as a record of the incident, the steps taken to determine vaccine viability, and the disposition of the affected vaccines. Keep this report for your records.
