This questionnaire will help the Palm Beach County Food Bank (PBCFB) assess if your agency meets the basic requirements to become a Partner Marketplace affiliate.
Once reviewed, a Program Coordinator will reach out to you.
Please note that meeting the minimum requirements does not guarantee approval of the partnership. Your EIN must be verifiable on the IRS website as tax-exempt.
Interested agencies must provide copies of the following documents.
- Letter of 501(c)(3) status (or equivalent non-profit status)
- DR-14 tax exempt form from the State of Florida
- Food Safety Certificate
- Proof of pest control (pest control of mobile unit if a mobile pantry)
- The program must already be in operation for at least six months
