WIOA Pre-Screening Form

CGTC WIOA Pre-Screening Form

WIOA is designed to help job seekers access employment, education, training, and support services to succeed in the labor market. WIOA is a federally funded program and is designed to serve dislocated workers, adults, and youth who need training to enter or re-enter the labor market. The first step in the WIOA process is to determine eligibility prior to completing the WIOA application. Eligibility is determined though the following pre-screening form. After completion of the pre-screening form, a staff member from WIOA will contact you to set-up a meeting to discuss your application options and request additional information as needed. To get started, complete this WIOA pre-screening form to the best of your knowledge. The information you provide will be kept strictly confidential. If you have any questions, please contact the WIOA department at wioa@centralgatech.edu or 478-476-5252.

We look forward to working with you!

Please note: WIOA is not an entitlement program; therefore, services are not provided to everyone. Our ability to serve you depends upon the following: Availability of funds to aid; Eligibility based on WIOA Selection policies & regulations; WIOA approved program of study; and Determination & qualifications necessary to graduate and obtain training-related employment. Final approval is made by the Middle Georgia Consortium.

You must reside in Baldwin, Bibb, Crawford, Houston, Jones, Monroe, Peach, Pulaski, Putnam, Twiggs, or Wilkinson counties to receive WIOA services through Middle-Georgia WorkSource Georgia Offices. If you do not reside in these counties, please contact the Worksource Georgia Career Center serving your county of residence.
