Please complete the following form to submit your nomination for the David Cooksey PestVet of the Year award. The selection committee asks you to provide as much detail as possible on your nominee's qualifications and contributions.
Nominations are due Wed. Feb. 9 of each year to be considered for that year's award. The winner will be honored at NPMA's Legislative Day. Please contact pestvets@pestworld.org with any questions.
Criteria
The evaluation process will recognize the following:
- The nominee’s leadership/involvement in community volunteer activities
- The nominee’s contributions to the pest control industry
- The nominee’s contributions to his/her company
- The nominee’s contributions to other Veterans
Minimum qualifications include:
- Must be an honorably discharged (DD214) Veteran of the US Military
- Must be employed by an NPMA member company
