We want your voice to be heard as we plan for the future! Please take 5 minutes to answer the following questions to help us improve our program. This survey is an opportunity for you to share your opinions and feedback on this year's program.
This program is funded by the 21st Century Community Learning Center Grant (21st CCLC). The 21st Century Community Learning Centers grant provides funds to districts and community-based organizations to provide enriching opportunities, including academic and well-rounded activities, during out-of-school time. When you
answer the questions in this survey, please think about the program and the activities that your child participated in after school, before school, or any other time they participated in program activities.
Please note, if you have more than one student participating in this after school program, please direct your responses for the student whose first name starts with the letter that is closest to the letter A; we only need one survey per family. This survey is voluntary. You do NOT have to answer any question if you do not want to, but we hope you will answer as many questions as you can.
No one can tell who filled out this survey. Your answers will be completely private and unknown to others. Your answers will be combined with those of other parents to help us navigate future programming to better support our families and students.
