This form is for requesting analyzed/aggregate health data from LMPHW only. If you are in need of other services, please call our main office at 502-574-6520, or dial 311.

Open records requests (such as inspection results) should not be submitted using this form. Instead, please submit your request through our open records request form.

• I will cite Louisville/Jefferson County Metro Government as a data source for all studies and other applications that use or rely on provided data.
• I agree to indemnify and hold harmless LMPHW and its employees, officers, and agents from and against any claim, cause of action, liability, damage, cost, or expense arising out of or in connection with any unauthorized or prohibited use or disclosure of provided data.
