- Brief statements on why you would like to join AHS as a member and how you can contribute to the organizational mission to advance the science and treatment of hernia.
- One sponsor endorsement from one of the following: AHS Member, Chief of Surgery, Chief of Staff, Fellowship Director or Supervisor
*Your sponsor will be notified by email and asked to confirm their endorsement of your application by selecting yes/no. Though letters of recommendation are not required, your sponsor may include optional comments for review.
