CLTS Council Representative Application, F-03408 (10/2025)

Are you interested in being a representative on the Children’s Long-Term Support (CLTS) Council? The Wisconsin Department of Health Services (DHS) is gathering information as part of the application process. We are looking for nominees who can make a commitment to be active participants on the Council during the term of the appointment.
