iC.A.R.E. Mentoring® Nomination Form

Please complete the online form fully when making this nomination. Please talk with the student about the iC.A.R.E. Mentoring® program and why you think the student might like to have a mentor. It is important to verify that the student wants a mentor before making the referral.

Only students who were absent 13 days or less in the previous school year are eligible for the iC.A.R.E. Mentoring® program. iC.A.R.E. Mentoring® program considers a student as absent if they miss more than half their periods per day.

We are accepting students from the districts listed in the drop-down menu below at this time. Charter schools are for J.O.G (Jobs for Ohio Graduates) students only.
