Thank you for taking the time to share with us the work you have completed so far this year. We are grateful for the work you do every day to promote oral health around the state and are looking forward to hearing about what you have accomplished.
The progress report is a requirement of agencies receiving funding from the Wisconsin Department of Health Services (DHS). This form, to be completed by all grantees, is designed to assess the current status and needs of the grant, address any problems or concerns regarding programmatic activities, or fiscal questions, and to share information/resources that may be useful to achieve grant goals.
Although you will be reporting your work on a biannual basis, the information you are sharing with us should reflect the activities you have completed from July 1 through the end of the current quarter. See the timeline below for more detailed information:
Mid-Year Reporting: Report on activities your agency completed from July 1 - December 31, reporting is due February 1.
Final Reporting: Report on activities your agency completed from July 1 - June 30, reporting is due July 31.
