SCORPIO HR ACTIV2D version 1

Thank you for responding to our ad for SARS-CoV-2 (COVID-19) treatment research!

Project ACHIEVE is looking for volunteers with recent COVID-19 to participate in research to treat infection of SARS-CoV-2, the coronavirus that causes COVID-19.

The purpose of the study is to evaluate the ability of an experimental oral drug called S-217622 to improve the health of people with recent COVID-19. We also want to see if this study drug is safe, and if this study drug can shorten the time you have COVID-19 symptoms, reduce the amount of virus in the body, and prevent hospitalization or death.

We are looking for people who are at least 18 years old and who, in the last 3 days, tested positive for SARS-CoV-2.

You will receive up to about $600 for your time and travel if you complete all visits. Private car transportation will be provided for your safety when you have active COVID-19.

You will be asked to answer a few questions on this survey to find out if you meet basic study criteria.  We will ask for documentation of laboratory test result(s) for verification. If you are eligible and interested in participating, you will be asked to provide contact information so we can ask you more questions by phone and possibly schedule an appointment.  If this initial appointment is scheduled, you will find out more about the study. If you are still interested in joining, you will continue the process to see if you are eligible for a study. You will receive compensation at each scheduled visit.

Your decision to take part in the survey is up to you. You may stop at any time and close your browser.  A risk of taking part in this survey is potential loss of confidentiality or privacy, but we will take every effort to avoid this. Your information will be stored on a secure server and will only be accessible by the study staff. The information will be saved indefinitely to assess study progress.

1. Click “YES” if you understand the above information and agree to complete this eligibility survey.
