Rhode Island - Fertilizer Usage

Informed Consent/Assent Form


You are invited to take part in a research study about lawn care and management on Aquidneck Island. The purpose of this study is to explore human decision-making. You will be asked questions about your opinions, beliefs, and preferences. If you agree to be in this study, you will need to make decisions and answer questions regarding the study materials. 

In addition to your opinions and beliefs, at the end of the study, we will also ask you personal demographic questions about you, including age, gender identity, ethnicity/race, level of education completed, household income, household size, marital status, current employment status, and political orientation. The survey platform automatically collects Geodata and IP address. We will not ask for your name, address, or any other information that will make you identifiable. We will however ask for your email address to be able to reach you with the prize if you win.

Results may include summary data of your responses to the study, but you will never be identified. We will not sell or share any identifiable information with any third party. We will securely store study data until the end of the research period.

Questions assessing attention will be placed throughout this study to ensure the validity of your responses. Failure to comply with the attention questions will result in the rejection of your participation. Please exit the study now if you do not agree. 


For your participation in this study, you will be entered into a draw to win a gift card for 100 USD (delivered via email in October). The risks of participating are no greater than those encountered in everyday life. Your participation in this study is completely voluntary, and you may refuse to participate or withdraw from the survey at any time.
