This survey is a
screening tool created by the Barry-Eaton District Health Department to screen individuals who are at a higher risk of being exposed, or those may have already been exposed to the Monkeypox Virus and are interested in vaccination. If you are eligible for the vaccine, you will be redirected to our appointment scheduling page at the end of the survey. You will be able to choose which BEDHD Office to visit on that page.
If you do not meet eligibility criteria at this time you will be asked for your contact information and added to our wait list. We will contact you via email when eligibility expands.
