2023 Teaching Excellence Award Nomination/Application Form

The purpose of AUPHA’s Teaching Excellence Awards is to recognize faculty from AUPHA member programs who have demonstrated exceptional accomplishments at the undergraduate, masters, or doctoral level. The Awards seek to honor, annually, AUPHA faculty across Faculty Forums who demonstrate teaching excellence within various disciplines that inspire and advance other faculty within AUPHA and beyond.

Instructions:
The nomination/application requires uploading several documents. If you do not have all the required documents at this time, complete as much as possible. When you submit the form, you will receive an email that contains a link that will give you access to your form, allowing you to upload documents or edit responses. 

The deadline has been extended. Final submissions must be completed no later than February 17, 2023, for consideration.

For example, the Cultural Perspective and Inclusive Excellence Faculty Forum is relevant for nominees for the Teaching Excellence Award in Diversity, Equity, Inclusion, Belonging, and Social Justice.
