Free Wellness Day for the Performing Artist

The event is Sunday, October 18th at HSS Florida only.

The deadline to complete this form is October 4, 2026 to request an appointment at HSS Florida for a dance specific health screening. Once you've completed the form, you will receive communication from a staff member on scheduling.

Participation is free. Registration is required for the wellness screening. All levels are welcome. Ages 18+ or must have a parent/guardian present
