PWCS McKinney-Vento Form - Students in Shelter & Transitional Housing 2023-24

Welcome to the PWCS MKV Form to Identify Students in Shelter or Transitional Housing Programs

NOTE:  This form is only for students/families who are residing in shelter or transitional housing.

It is vital that the PWCS McKinney-Vento (MKV) Office identify each student in shelter and transitional housing programs in order for the student to receive educational benefits.  Students in shelter and transitional housing programs automatically qualify for McKinney-Vento.  The PWCS MKV Office needs to know basic information about the students/family, and this form has been simplified to make the identification process quick and easy.  The parent/custodian or school/housing staff can complete/submit this online form.  We want to give the student every benefit that they qualify for in school!

The PWCS McKinney-Vento (MKV) program strives to offer school stability for the given school year and provides the following educational benefits/rights to identified students:
  • School transportation (if the McKinney-Vento team determines it to be feasible) for students who remain in their school of origin while residing outside of school boundaries
  • Removal of enrollment barriers (i.e., no residency proof for enrollment) in order to provide immediate school enrollment and attendance
  • Automatic approval for free school meals for the school year
  • Participation in school activities

IMPORTANT INFORMATION REGARDING MCKINNEY-VENTO ...

The student's McKinney-Vento status is valid only for the school year in which the student is approved. A student/family must re-apply or contact the McKinney-Vento Office to be re-assessed for the next school year's MKV program if the student's housing situation appears to be temporary due to hardship.

MKV approved students have the rights to the following ...
 
  • Students do not need a permanent address or residency proof to enroll in school.
  • Students cannot be denied immediate school enrollment when school records or other enrollment documentation (birth certificate, immunization record, physical) are not immediately available. However, parent/caregiver may be required to complete a Birth Certificate Affidavit if there is no birth certificate available at time of immediate enrollment. From time of immediate enrollment, parent/guardian/caregiver has 60 days to produce immunization/physical and has 90 days to produce birth certificate. If those deadline dates are exceeded, the student may be withdrawn until the parent/guardian/caregiver can produce the required documentation.
  • Students may be able to remain in the same school (school of origin) even if the student moves outside of school boundary or into permanent housing during the school year.
  • If the MKV Liaison determines it to be in the student's best interest, he/she has the right to transportation services (if deemed to be feasible) to and from the school of origin.
  • Students are automatically added to the school's free meal programs for the school year.
  • Students have the right to participate in extracurricular activities and all federal, state, or local programs for which he/she is eligible.
MCKINNEY-VENTO HOMELESS EDUCATION SERVICES AGREEMENT ...
 
  • The parent/custodian/caregiver are aware that approved student(s) will receive MKV services/transportation through the remainder of the given school year even if the student/family obtain permanent housing (IF the McKinney-Vento team determines it to be feasible.)
  • The parent/custodian/caregiver understand that it is the parent/custodian/caregiver/HUY student responsibility to update the school office with any changes in address or contact information.  The parent/custodian/caregiver/HUY student will provide the school with a good mailing address which might not be the address where the student/family is residing due to reasons of confidentiality or risking others housing situation.
  • The parent/custodian/caregiver understand that the student(s) has the right to participate in extracurricular school activities which includes sports; however, the McKinney-Vento Act does not explicitly require that transportation be provided to/from extracurricular activities.
  • The parent/custodian/caregiver understand that if the student(s) does not maintain regular/on-time attendance, good academic performance and/or exhibit good behavior in school on a regular basis, a best interest determination meeting may be held to discuss transferring to the school that is zoned for the student's residence. The parent/custodian/caregiver also understand that I would have the right to appeal a decision and the MKV Liaison will supply me with that information if requested.
