Employee Exit Interview

Introduction

We want to know more about your impressions as an employee of Three Rivers District Health Department. As such, we greatly appreciate your time and efforts to respond to this survey. While none of the questions are required to be answered, we ask that you thoughtfully consider and answer all the questions. Doing so will help aid our future efforts in recruitment and retention of employees.

The survey generally takes approximately 10-15 minutes to answer all the questions. As your candid answers are greatly valued, please know that they may be shared with those having a business need-to-know so TRDHD can continue to grow and be the best workplace it can be.   • Employee demographics are collected solely for the purpose of helping to identify specific areas/roles that may be associated with better or worse employee satisfaction and retention outcomes. None of your responses will be associated with your personnel or performance record.
  • Employment experience and reasons for leaving questions pertain specifically to your experience in your most recent role at Three Rivers. Please answer with as much detail as possible. Your input will ensure we have the information we need to make improvements and thrive as a local employer.
  • New employer information will help us to identify our competitors for the best and most qualified employees. This information is used solely for the purpose of improving Three Rivers hiring and retention of employees and will not be used for future contact or solicitation.
