Informed Consent

Welcome, this is an epidemiological survey and is intended only for healthcare professionals.

Pressing the forward button implies your informed consent, that you are above 18 years of age and that you are willing to participate in this study. The study aims to identify risk factors and characteristics of pain and chronic pain among healthcare professionals.
The survey is completely anonymous and does not require leaving any personal details 
If you wish not to participate, you may leave now or at any point during the survey.
The study aims to assess the level of pain and chronic pain among healthcare professionals.
The time to fill the survey ranges from 5 to 8 minutes.
