The Australian Primary Health Care Nurses Association (APNA) is seeking Expressions of Interest for the next intake of the Chronic Disease Management & Healthy Ageing Program

This information outlines some of the things that we need to let you know about as part of better practice in evaluation, contact details if you have problems as well as practical information around frequently asked questions such as “How do I go back, “How do I save my survey?”, “What does this error message mean?” and “Do I get to keep a copy of my survey?”
