Well-being Inventory

We’ve designed a novel inventory of well-being to help you get real clear on how you’re doing in mind, body, life, and work.  Everyone has a unique groove; we want to help you find yours.

Your individual answers will be kept confidential and will only be seen by Wellcoaches staff for professional use.  If these data are used in a presentation, you will not be identified and your personal information will not be shared, but only used in a group format.

For each statement, please answer using the numbers 0 to 10, where 0 means “not at all” and 10 means “completely”, to indicate how true it is of you.  When considering each statement, please choose the answer that best describes your own opinion of what is generally true for you.
