Florida Dam Information Survey

Applicability

If you are the owner or owner’s representative of a dam located in the State of Florida and it meets at least one of the criteria listed below, please provide information for your dam. This survey does not apply to a levee or levee system. There are no requirements associated with providing this information.

Provide information for dams that meet one or more of the following criteria:

• Dam failure (breach) or mis-operation (unscheduled release) would likely result in one or more of the following: loss of human life, offsite economic loss, environmental damage, disruption of lifeline facilities, or impact other concerns, such as, water quality. 
• Dam height is greater than five feet.
• Maximum storage is greater than 15 acre-feet. 

Instructions 

Open the Dam Parameter Definitions (link will open in a new tab or browser window) to complete this survey . The definition and units of measurement for each bolded parameter in this survey are defined in the Dam Parameter Definitions document
