Camp Pierce Required Registration Information 2023

4. Does the camper have epilepsy or any kind of seizure disorder? *This question is required.
5. Has the camper had a seizure within the last year?
5. Is the camper taking medication of any kind? (This information will only be used in case of an emergency and a medical response is required.) *This question is required.
6. Will the camper need to take any medication during camp hours? *This question is required.
8. Are there any other special precautions or any life-threatening health concerns we should be aware of? *This question is required.
9. Does the camper have a Regional Reduced Fare Permit (RRFP) card for transit?  *This question is required.
10. Does the camper sunburn easily? If so, please send spray-on sunscreen daily. *This question is required.
11. Can the camper dress and change clothes independently? *This question is required.
12. Will the camper take and follow basic instructions and/or directions? *This question is required.
13. Is the camper able to read independently? *This question is required.
15. Can the camper ambulate (move using feet, walker, wheelchair, etc.) independently for a distance of at least two city blocks without needing to stop? *This question is required.
16. Does the camper use any special equipment to ambulate (walker, wheelchair, crutches, cane, etc.)? *This question is required.
17. Camp Pierce will be out and about in the community the majority of the time and bathrooms will not always be immediately available. Is the camper able to identify and indicate that they need to use the bathroom at least 15 minutes before it becomes an "emergency"?  *This question is required.
18. Will the camper use the bathroom when instructed? For instance, if instructed will they use the bathroom prior to leaving the community center? *This question is required.
19. Can the camper use the bathroom without assistance. *This question is required.
