Daily Health Screen

To prevent the spread of Coronavirus and reduce the potential risk of exposure to others, all employees, students, and visitors are required to complete and submit this daily symptom assessment before entering the school or during the first 15 minutes of entering any of the school property. 

Please answer each question truthfully and to the best of your ability. This assessment is the first step for us to take precautionary measures to help you and fellow co-workers and classmates beat the pandemic.

Have you tested positive for COVID-19 in the last 10 days?
Are you currently waiting on results of a COVID-19 test because of possible COVID-19 exposure, such as within close contact of a COVID-19 confirmed positive case?
Within the past 14 days, have you been in close physical contact (6 feet or closer for at least 15 minutes without a mask) with a person who has received a positive test result for COVID-19 or anyone who has symptoms consistent with COVID-19 within the past 14 days?
Have you experienced any of the following symptoms that might be related to COVID-19 in the past 48 hours? These are fever/chills, cough, difficulty breathing, fatigue, muscle or body aches, headache, loss of taste or smell, sore throat, congestion or runny nose, nausea, diarrhea, or vomiting.
A "YES" answer to any of the question above requires you to IMMEDIATELY STAY AT HOME for 10 days and isolate.  DO NOT ENTER any JJS Castleson campus or property to prevent the spread of COVID-19.  For questions, immediately contact the Academic Director, Ms. Cora, or email vpjjscastleson@gmail.com.
