IMPACT Youth Mentorship Mentee Application

Welcome!

This application must be completed by the parent or guardian of the perspective child/youth participant. The purpose of this application is to help Youth Emergency Services IMPACT Youth Mentorship Program know more about the mentee. The IMPACT Youth Mentorship offers two types of mentoring:


Community Based Mentoring: Mentors provide your child with a positive role model and a strong support system. Mentors in this program must be 21 years or older and commit to meet with their mentees for a minimum of one hour per week for a minimum of one year. They complete a screening process, background check and participate in required trainings. Mentors and mentees are matched based off of interest, preference, needs of mentees and qualities of mentors. They will participate in many activities within the community and gain a wonderful friendship. Any students, Kindergarten through 12th grade are eligible to apply for a Community Based Mentor.

Site Based Mentoring: Mentors provide your child with a positive role model and a strong support system while they are participating in school. Mentors commit to meet with their mentees at our participating schools after school for one hour a week. Mentors in this program must complete a screening process and complete all required trainings. Mentorship staff are always present at site based programs. Mentors and mentees are matched based on interests, needs of mentees and qualities of mentors. Mentees in this program can participate in various activities within their school and will gain a wonderful friendship with their mentor.
