OIS-G tuition request form for Personal Support Workers Program Information
The Oregon Home Care Commission (OHCC) will review the completed form to determine if the Personal Support Worker (PSW) qualifies for the OIS-G tuition to be paid for by the Oregon Home Care Commission.
The PSW should complete this electronic form. Once the application is submitted, OHCC will automatically receive the results. Applicants can expect to receive a follow-up email in about a week after submission.
PSWs will be approved if they meet the following requirements:
1. Active provider number; assigned to provide services to a person who has a Positive Behavior Support Plan in place and
2. The Personal Agent or Service Coordinator agrees that the PSW needs the OIS-G training. PSWs will be approved for one time during the certification period.
or
3. The Personal Support Worker has completed two OHCC courses within the last 2 years, excluding OIS-G.
How it works
If a PSW is approved for OIS-G tuition coverage, the PSW will receive a confirmation email from OHCC. This may take approximately a week to approve. We will be reaching out to the Service Coordinator or Personal Agent. Once you are approved, you may locate a workshop in your community by referring to the Workshop calendar visible on the ASI Oregon website.

You will need to notify us, once you find a workshop you want to attend, and that workshop instructor agrees to accept PSWs with payment being from the State of Oregon. We will notify you if the instructor agrees to the tuition. You may then attend the course. In some cases, we can start by working with an instructor and the PSW will sign up for one of their workshops; this depends on the availability of instructors in some parts of the state.

2. Who does not qualify for OHCC Pre-Approval for OIS-G course tuition?
• PSWs who are pending approval for their Medicaid provider credentials
• HCWs (or other types of providers) not enrolled in the CIIS or DD-PSW programs
3. What is a stipend?
• A stipend is a payment OHCC issues to PSWs who meet the qualifications
• Each stipend is based on the total number of hours completed during the approved training
• OHCC will verify the information before determining if the PSW qualifies for stipend payment
4. Who qualifies for an OIS-G stipend?
• Only PSWs who:
- have an active Medicaid provider number;
- are employed by a person receiving Medicaid in-home services;
- completed their OIS-G certification;
- are required to have the certification as stated on the approved Positive Behavior Care Plan, and;
- their information has been verified by OHCC with the approving Personal Agent or Services Coordinator
