All team members entering a Spectrum Health facility to perform work or provide any in-person patient care today, must check symptoms and risk factors before each shift via the screening survey. The survey must be completed prior to your shift and is available four hours before your start time.

Regardless of survey results, if you feel you have ANY symptoms related to COVID-19 infection, for the fastest screening and testing:

PH/Corporate/WM team members please use MyChart through the Spectrum Health app. If you do not have a MyChart account, one can be created through the app. Alternatively, you can call the Spectrum Health Team Member Hotline at 833-559-0658.

Any individual entering a Spectrum Health facility to perform work (consultants, designers, contractors, subcontractors) must complete a daily screening survey. The survey must be completed prior to your shift and is available four hours before your start time.
Individuals should only be entering the facility if they receive a green check. Please be ready to show proof of completing the daily survey at any given time.
Enter through approved/established checkpoints and follow proper PPE Guidelines (isolation mask). Hand sanitizer and masking stations are set up at each employee/patient entrance.
DO NOT USE Emergency Department entries as access points
If you do not know the approved entry point, please consult with your Spectrum Health project manager on approved locations to enter any Spectrum Health facility
