CfY Mentor Application

Thank you for your interest in becoming a mentor with Community for Youth!

Please review the complete application and screening process below:

Step 1: Submit this application. It will take about 40 minutes total to complete.
Have the following documents ready:

Driver's License
Auto Insurance
Driving Record (cost $13)
- Visit this website
- Log in or join License eXpress to buy a copy of your own driving record.
- Request the record labeled as "Employment"

Step 2: Complete background check after we review your application. (cost ~$40)
Step 3: Schedule an in-person interview with CfY Staff

Bring Proof of COVID-19 vaccination

Step 4: Complete Mentor Training
Step 5: Staff will review all your materials and if accepted, you will receive an acceptance email and welcome packet, including: workshop calendar, staff contact information, FAQs

All students & mentors are required to be vaccinated, which means having both primary shots. It is highly encouraged, but not required, for everyone to receive all booster shots as well. It is the policy of Community for Youth to not accept applications from candidates who are not fully vaccinated. *This question is required.
