To apply for one of these awards, please complete the following application, including a 300-500 word essay answering the following questions:
"Why are you interested in the health career of your choice?" and
"What have you done to prepare for applying to your professional program?" For assistance with reviewing your essays, please contact the
writing center. You will also be asked to submit contact information for a Cal Poly faculty reference. If you have a resume, you will have an option to upload it. Refer to
career services if you need assistance with a resume. A resume is not required, but can help the selection committee in making a final decision.
Applicants will be reviewed by quality of essay, scholarship (GPA & faculty reference), resume of experiences (if applicable), and demonstration of financial need as determined by the financial aid office. Financial need is not a requirement for applicants, however demonstration of need is a factor considered in the selection process.
By submitting your application, you authorize permission of the selection committee to confirm your financial need with the Cal Poly Financial Aid Office based on your submission of the FAFSA or CA Dream Application and receipt of financial aid as determined by the needs analysis calculation.
Decisions will be made and recipients will be notified by
Monday, April 4th.
If you have further questions about the essay contest, please contact Kristi Weddige at
