Digitization Kit Reservation Form

Use this form to submit a request for reserving one of the Library of Michigan's Digitization Kits. This project was designed to increase the capacity of Michigan's libraries and cultural heritage institutions to digitize, provide access to, and preserve their local history collections through digitization. 

Each kit contains everything needed to digitize common formats; please review the contents for each to select the kit appropriate for your project. 

This project was made possible in part by the Institute of Museum and Library Services.
Kits ship via UPS and cannot be delivered to P.O. Boxes.

8. Is this kit going to be used for a community scanning project outside of your organization? *This question is required.
9. The Library of Michigan is expanding its CONTENTdm instance to host digital files from external contributors in order to reduce barriers to sharing information about the state. Are you interested in having your collection(s) hosted on the Library's CONTENTdm? *This question is required.
10. The Digital Public Library of American (DPLA) aggregates metadata from online collections in order to amplify content from trusted repositories. Are you interested in contributing to the DPLA's Michigan Service Hub to increase access to your digital collections? *This question is required.
