Overview
The Disability Vaccine Access Opportunities Center is operated with funding granted by the CDC Foundation and awarded to Able South Carolina. The principal collaborator with Able South Carolina is ILRU (Independent Living Research Utilization). Funding for this effort is supported by the Centers for Disease Control and Prevention of the U.S. Department of Health and Human Services (HHS).
Awards of up to $50,000 are anticipated.
Instructions to Applicants
To apply, please submit your application and upload the following documents
no later than 12:00 pm (Noon) EDT on Monday, January 24, 2022:
- Attestation Document affirming funds will only be used for COVID-related activities performed in unserved counties. (Part C CILs only)
- Budget and budget justification (see example budget and budget justification in Appendix A of the RFA)
- Most recent annual budget
- Grant applicants must provide one of the following three financial documents at the time of application:
- Most recent Transmittal Letter for the organization’s annual audit
- Most recent IRS Form 990 (Return of Organization Exempt from Income Tax)
- Most recent year-end financial statement (if applicable)
- IRS determination letter - letter from the IRS confirming 501(c)(3) status, or the verification provided from the IRS website. (Organizations that have received 501(c)(3) status are exempt from federal taxes. To receive this status, the organization must operate for a specific purpose--typically, for a charitable, religious, scientific, or literary purpose. Should your organization need assistance with this information, please contact the IRS at 877-829-5500. (There is no cost to obtain this information from the IRS.).
- Certificate of Liability Insurance by date of the award
- Form W-9
- Vendor status verification: U.S. System Award Management (SAM), or equivalent, search results for overall organization and individuals designated to this proposal. All applicants must have a DUNS number (https://fedgov.dnb.com/webform/) and be registered with the System for Award Management (SAM, www.sam.gov) and maintain an active SAM registration until the application process is complete, and should a grant be made, throughout the life of the award. Applicants should allot sufficient time prior to the application deadline to finalize a new, or renew an existing registration. This action should allow you time to resolve any issues that may arise. Failure to comply with these requirements may result in your inability to submit your application or receive an award. Maintain documentation (with dates) of your efforts to register or renew at least two weeks before the deadline. Eligibility Criteria *
To apply, the applicant must be a Center for Independent Living. Centers for independent living are consumer-controlled, community-based, cross-disability, nonresidential, private nonprofit agencies for individuals with significant disabilities (regardless of age or income) that are designed and operated within a local community by individuals with disabilities and provide an array of IL services as defined in section 7(18) of the Rehabilitation Act as amended by the Workforce Innovation and Opportunity Act (WIOA).
Applications for funding will be accepted for review from organizations that meet the criteria specified below.
