As part of the Comprehensive Cancer Control Program's (CCCP) commitment to advancing health equity, we are collecting success stories. CCCP in partnership with the Pennsylvania Cancer Coalition (PCC) have developed an online
Health Equity Achievements tool that provides grantees and partners with a way to show why the work matters and how funding is used.
PA Cancer Health Equity Achievements are accounts that share positive results and ongoing success of health initiatives.
The database was developed to help meet the Healthy People 2030 objective to “increase the number of health departments that use social marketing in health promotion programs” and to “evaluate the impact of using social media to raise awareness about health equity and effective interventions that address health equity/health disparities in cancer control.”
Submissions are evaluated on the following criteria:
- Describe the issue being addressed and explain its scope with specific metrics.
- Outline the details of the project, including key collaborators, the timeline, the population of focus, and how progress was measured.
- Report the results and key takeaways from the initiative.
Please complete the following form so we may evaluate your submission. All submissions should include a succinct overview of the program, identify and describe the problem solved and present the outcomes of the intervention utilized.
