The Minnesota Department of Education (MDE) administers certain USDA Child Nutrition Programs including School Nutrition (SNP), Child and Adult Care Food Program (CACFP), Special Milk Program (MILK), and the Summer Food Service Program (SFSP). This survey should be completed for organizations or schools that are ready to submit an application for these programs. If your organization is not ready to submit an application, please review the
MDE website for more information about these programs.
This form is the initial submission of information and intent to apply for SNP, CACFP, Milk and SFSP. MDE will use the information provided to pre-screen new applicants to ensure eligibility for the USDA Child Nutrition Programs. MDE will also use the information to create your organization's access to the Cyber-Linked Interactive Child Nutrition System (CLiCS). Once you receive access to CLiCS, your organization can submit the sponsor and site applications for the program you designate. Once submitted, the application will be routed to MDE staff for assignment and an MDE staff person will contact you. Depending on the Program, there may be other enrollment requirements, for example CACFP may require a management plan.

Data entered on this form cannot be saved. All applicants should have the following information available before you begin:

- MDE will use the information to match to the various identification numbers and MMB Supplier portal. Please make sure the name and addresses provided match the information registered to your tax ID numbers, Unique Entity Identifier, and MMB supplier/vendor number.
- Federal Tax ID Number IRS Employer Identification Number
- Unique Entity Identifier. The (UEI) ID is a 12-character alphanumeric ID assigned to an entity by SAM.gov. The DUNS Number has been removed from SAM.gov. SAM.gov now require use of the new Unique Entity ID.
- Existing registered entities can look here to find your Unique Entity ID.
- Quick Start Guide. New entities can get their Unique Entity ID at SAM.gov and, if required, complete an entity registration.
- Minnesota Department of Management and Budget-SWIFT Vendor Resources - MMB Supplier/Vendor Number (this is the number used to provide the reimbursement to your organization)
