Each month, you are required to use this survey to report the number of UNDERinsured Texas Vaccines for Children (TVFC) clients your facility administered vaccine to, and the number of doses administered. This data is due on the 15th of each month and should contain data for the prior month. For example, if you are reporting data for January 1st-31st, you will report these numbers by February 15th. You cannot report multiple months using one survey.
UNDERinsured is defined as:
- A child who has commercial (private) health insurance, but coverage does not include Vaccine
- A child whose insurance covers only selected vaccines (TVFC-eligible for non-covered vaccines only)
- A child whose insurance has a fixed dollar limit or cap for vaccines (TVFC-eligible once fixed dollar amount or cap is reached).
