Riley County Health Department's Medically Vulnerable Population Form

Riley County Health Department Medically Vulnerable Population Registry

The Special Medical Needs Registry is an emergency preparedness tool that can be used by Riley County Health Department, Emergency Management, Emergency Medical Services (EMS) and Riley County Police Department (RCPD) for communicating with medically vulnerable and fragile individuals during pre-event planning and during emergency response operations. The Riley County Special Medical Needs Registry is a tool that aims to reduce adverse impacts on vulnerable populations throughout Riley County and aid planning and response entities in ensuring needs are met during times of disaster.

Registration is voluntary.

This information is used for planning and emergency response. It will not be shared with outside entities, and will be kept confidential

This does not take the place of 911 in the event of a life-threatening situation. If in an emergency, call 911.

The special medical needs registry helps public health, emergency management, and emergency medical services personnel know who may need extra help during an emergency. This information helps officials plan for safe evacuation, and to ensure safe and adequate shelter environments for those with special medical needs.

Who Should Sign Up for the Registry?

People with special medical needs include those who would need assistance with medical or personal care during evacuations and sheltering because of physical or mental impairments. Sign up for the registry if you or a loved one:
