National Engineering Crest: Post-Event Program Evaluation

Now that you have completed your engineering activity, we appreciate you taking the time to provide some information about your event.  Your feedback allows us to improve the program, continue to provide the crests at no charge, and better serve your needs.   Note: Crests will be mailed once the completed evaluation form is received and reviewed.

When filling out the form to request the crests, you must consent to Engineers Canada collecting and using your personal or other information in accordance with its Privacy Policy, which may include disclosing to engineering regulators and/or any other participating third parties and other volunteers. Use of your Personal Information
The personal information collected is only used by Engineers Canada staff for the purposes defined at the time of the collection or a use that complies with these purposes. Engineers Canada will share your personal information with third-party service providers who require such information to assist Engineers Canada with administering the Girl Guide Crest Program, including third-party service providers who provide services to Engineers Canada or on its behalf. Third-party service providers include, but are not limited to the twelve provincial and territorial engineering regulators and Girl Guides Canada.
