Please have all of the relevant information regarding any positive individuals, household contacts, and any other close contacts (beginning two days before symptoms started or two days before a positive test was collected) before beginning the survey. If you or anyone else took an at-home test, please take a photo of the test card in order to upload it as a part of the case.
